Permission and Waiver
When you register a child for St. LouisPark Community Edcuation classes and activities, you agree to allowyour child to participate in the activity. For yourself and your heirs,you waive any and all rights and claims for damages you may haveagainst ISD #283 or the City of St. Louis Park and its representatives,for any and all injuries from whatever cause suffered by the aboveparticipation in the indicated activity. In case of emergency, thestaff will use their judgement with regard to treatment until you canbe contacted. The information that you have provided will bedistributed to individuals involved with each program. |
